PRIVACY POLICY

Here at Hollinton Health, our teams are dedicated to keeping our customers and staff safe. We have strict disinfection procedures in place, leaving sufficient time between customers to perform these, and are wearing full PPE to protect ourselves and others. Appointments will initially be shortened to 30 minutes (not including initial online consultation) until restrictions are lifted.

When sessions resume 15th June, there will be an additional declaration form to complete stating that:

a) You not been in contact with anyone with Covid-19, in the last 14 days, to your knowledge
b) You have not had any symptoms: - dry cough, temp over 37.8°C, loss of smell and/or taste
c) Should you contract the virus you must inform the therapist as soon as possible
d) Should you contract the virus we are obliged to inform NHS Track & Trace


There will also be a signed declaration from the therapist stating the same declaration

All first-time and follow-up consultations will be carried out either on the phone or via video conferencing to determine the need for face to face appointments.

Sports Therapy 4U Ltd is a specialised sports injury rehabilitation clinic based at Hollinton Health, Baldock, Hertfordshire. Our team comprises of sports therapists, injury rehabilitators and sports massage therapists.

Wellness Therapy 4U Ltd is a wellness therapy clinic based at Hollinton Health, Baldock, Hertfordshire. Our team comprises of counsellors, Acupuncturists, Holistic therapists and foot healthcare practitioners.

WHO DO WE COLLECT DATA FROM?

We collect and process data from a range of individuals, including those that have become clients at our clinic, those that have expressed a strong interest in becoming a client, those that have used or have expressed a strong interest in using our services at sponsored events and those that have visited our website. We also collect information when you voluntarily complete customer surveys, provide feedback and participate in competitions.

WHAT INFORMATION DO WE COLLECT ABOUT YOU?

When booking an appointment, we collect and process first name, last name, date of birth and postal address under the lawful processing of contract. E-mail and telephone number are collected under the lawful processing of legitimate interest. This information is required to monitor your activity with us and send appointment reminders. We also collect and process medical records of your treatments which are kept for 7 years (minors information will be kept 7 years after they turn 18) in compliance with our insurance policies (Contract law).

We also collect information about your past injuries, current injury (if any), health and fitness goals and any relevant medical information when attending an appointment. This is to ensure your health and safety when using our services and is also processed under the lawful processing of legitimate interest.

HOW WILL WE USE THE INFORMATION ABOUT YOU?

The information collected when you attend our clinic or one of our sponsored events is required to ensure you get the most benefit from our services. The data will be used to set up, verify and manage your progression, create tailor-made programmes, allow you to track your progress and to verify what treatment you have undertaken.

The information collected on behalf of a child using our services is required to set up their progression. Parent/carer details are required to verify and manage this activity on their behalf.

We collect data on the facilities you have used to keep you updated by text, email and/or phone on relevant matters such as terms or bookings changes, booking confirmations, and important updates regarding the clinic and changes.

We will also contact you via email, telephone, text and/or post about our other products and services we think may be of interest to you. You will have the option to opt-in to these when you initially provide data to us or when we collect data from you, and can opt out/update your preferences at any time.

We use information collected from the website to get an idea of best performing web pages.

If you have used our facilities, we will keep your data after your booking has ended. After this, your data will be archived for seven years and then securely disposed of.
